Create and share an intake form

Publish a form, then create its sharing link.

   Create a draft

   Open Forms, choose New form and give it a name. Add the questions you need.

   Publish a version

   Save draft, review the questions and select Publish version.

3. 3

   Create the link

   Open the sharing controls and choose Create sharing link. Copy it and review responses in Forms.
